What is C-section scar endometriosis?

If you have a painful lump in or near your caesarean scar that becomes more painful with your periods โ€” this is almost certainly C-section scar endometriosis (also called parietal or abdominal wall endometriosis).

During caesarean section, endometrial cells can implant in the scar and grow cyclically under hormonal influence, forming a nodule that swells and hurts with each menstrual cycle. It typically appears 1โ€“3 years after the caesarean and will not resolve spontaneously.

Recognise the symptoms

  • A firm, tender lump in the C-section scar โ€” visible or palpable
  • Pain that worsens during and around your period โ€” the hallmark sign
  • Sometimes swelling or bruising of the scar during menstruation
  • The pain does not improve with time โ€” it typically worsens over years
  • Confirmed by MRI (T1-hyperintense nodule) or ultrasound

Treatment: cryoablation at Cochin AP-HP

Cochin AP-HP is one of the pioneering centres worldwide for cryoablation of C-section scar endometriosis โ€” one of the largest and most experienced teams globally for this technique. Under CT guidance, a cryoprobe is inserted through the skin directly into the nodule. Freezeโ€“thaw cycles at โˆ’40ยฐC destroy the nodule without any incision. 80โ€“95% of patients are pain-free or significantly improved.

Performed under general anaesthesia (30โ€“40 min), as a day case. No scar. No additional incision. Return to normal activity in 5โ€“7 days.
